The Home and Community Care (HACC) Program targets frail aged, younger persons with a moderate to severe disability and their carers to assist them to enhance their independence in the community and avoid premature admission to residential care. Assessment at entry point is required.
The Community Options Program (COP) targets frail older people who have complex needs and require the assistance of a case manager to access support to remain living in their own homes. A comprehensive assessment is required.
The Community Aged Care Packages (CACP) Program targets frail older people (generally 70 years and over) living in their own homes who require management of services because of their complex care needs. To access a CACP, a person must be assessed and approved by an Aged Care Assessment Team (ACAT).
The National Respite for Carers Program targets relatives and friends caring at home for people who are unable to care for themselves because of chronic illness, disability or frailty. The broad target group for the NRCP are primary carers. Specific target group for ECCLI NRCP services are carers from culturally and linguistically diverse backgrounds
Anyone can refer a person in need for our services with their consent.
Referrals are accepted from a variety of sources including yourself, your GP, family and friends, other health professionals and service providers.

Case Managers will contact then visit you after referral to our services have been accepted. This depends upon availability of resources and waiting list.
Your needs will be assessed and you will be notified in writing of the outcomes of this assessment and whether you have been admitted to any of our programs. A negotiated service agreement will then be signed by both parties.
