  • Homes and Community Care (HACC) program
  • The Home and Community Care program aims to assist frail older people, younger people with disabilities and their carers who need basic support services to continue to live in the community and prevent premature entry into a residential care facility.

  • Community Options
  • Community Options is a case management service for people whose needs cannot be met by a mainstream service. The Community Options approach begins with a holistic and comprehensive assessment of the individual person needs. A designated case manager works with the individual to develop a care plan that may involve other service providers, private agencies, family or neighbours. The Community Options focus is to plan and coordinate services around the individual in a way that ensures flexible, personalised service delivery. This program has a cultural component for people from non English speaking background who have additional needs and special characteristics.

  • Community Aged Care package
  • A Community Aged Care Package is a care option that provides an alternative to residential care. The CACP provider is responsible for organising community services for frail older persons with complex care needs who prefer to remain at home. The package is planned and coordinated by the CACP provider, who puts in place agreed services after a comprehensive assessment.

    Individual packages vary in the services they provide, and in their cost.

  • National Respite for Carers Program (NRCP)
  • The National Respite for Carers Program is one of several initiatives by the Department of Health and Ageing to support and assist relatives and friends caring at home for people who are unable to care for themselves because of chronic illness, disability or frailty.

    Respite Care is delivered to carers and the people for whom they care in a variety of settings which best meet their needs. The broad target group for the NRCP are primary carers. Specific target group for ECCLI NRCP services are carers from culturally and linguistically diverse backgrounds of:

    • people with dementia
    • people with dementia and challenging behaviour,
    • frail older Australians (65 years or over, or 50 and over if Indigenous),
    • younger people (under 65, under 50 if Indigenous) with moderate, severe or profound disabilities who are living at home, and
    • people with a terminal illness in need of palliative care.
